A Nurse Navigator at Illuminate is a critical team member to serve our customer base. The Nurse Navigator will be working with our Discovery Services team to provide at 360-degree approach to caring for patients with various incidentally identified disease processes including but not limited to, vascular diseases. The nurse navigator works with the client to facilitate customized workflows for specialty areas prior to “go-live.” The nurse navigator also facilitates communication and coordination of care between primary care, specialty care and various hospital departments. The nurse navigator provides oversight to clients who provide internal navigation to ensure success of the program.

Responsibilities:

• Performs quality review of patients identified by our software including but not limited to patient chart reviews
• Facilitates the ongoing coordination of care for patients with incidental findings, including communication with key members of individual patient care team
• Responsible for notifying either the patient or his/her provider when disease state is off surveillance protocol
• Accurately document all aspects of care coordination and follow up in patient management system
• Establish and maintain positive relationships with all involved providers and serve as liaison between various health system departments at client site 
• Perform a task called “data-labeling” to assist in developing future disease state machine learning models
• Travel to client sites to work directly with clinicians.  Travel requirements are approximately 4-8x per year
  

Qualifications:

• Bachelor’s degree in Nursing is required
• KS or MO nursing license required, ability to obtain and maintain multistate license and/or single state licenses in various states as needed
• 5 years acute care nursing experience required
• Excellent written and verbal communication skills 
• Excellent customer service skills
• Computer literacy required
• Leadership experience preferred
• Specialty nursing certification preferred
